For patients without insurance, my cash-pay rate is $160 per session.
What are your rates?
I accept most major insurance plans, and the cost of services depends on your specific insurance coverage. Your rate will typically be based on your co-pay as determined by your insurance provider. If you have questions about your co-pay or coverage, please contact the number located on the back of your insurance card.
What is your cancellation policy?
To ensure quality and fair care for all my patients, I require a 24-hour notice for cancellations or rescheduling. This policy allows me to offer appointments to others in need and maintain an efficient schedule. I understand that emergencies happen. If you’re unable to provide 24-hour notice, please contact me as soon as possible through the messaging portal on the platform where you booked your appointment or call me directly. I will do my best to assist you in rescheduling or accommodating your situation.
Please note that missed appointments or late cancellations (less than 24-hour notice) are subject to a fee of $80.
What are your office hours?
My office hours are Monday through Thursday, 8:00 AM to 4:00 PM. During these hours, I am available for appointments, questions, and support.
What if I need assistance after hours?
While I strive to address all patient needs promptly during office hours, CADP INC does not provide 24/7 crisis intervention services. Here’s how to handle various situations:
Non-Urgent Matters:
For non-urgent questions or to reschedule an appointment, please leave a message on the platform you use to communicate with me, or call my office. I will respond on the next business day.
Prescription Refills or Non-Urgent Medication Questions:
Please submit refill requests or medication-related questions through the message portal. If submitted after business hours, they will be addressed the following business day. Requests sent after Thursday at 5:00 PM will be processed on Monday, as my office is closed on Fridays and weekends.
Emergencies:
For emergencies requiring immediate attention, such as self-harm, danger to self or others, severe medication reactions, or psychiatric crises, please call 911 or go to the nearest emergency room. You may also contact the 988 Suicide & Crisis Lifeline by dialing 988 for 24/7 confidential support.
What about paperwork or forms?
If you need paperwork or forms completed or signed, please allow 4-7 business days for processing, starting from the date the request is submitted. A fee will apply based on the type of paperwork required. For more details, feel free to contact me directly.
